Your views and feedback are of great importance to the practice and help us to ensure that we are providing the best quality service and information to our patients.
To obtain the necessary data and opinions, we are planning to carry out a patient satisfaction survey each year, usually in Autumn, but sometimes at other times, to see how we are doing. To achieve this, we would like to create a patient participation group, the members of which have given their consent to be contacted periodically by letter, or preferably, by email. It is likely that the information we require will take only a few minutes to provide.
You can agree to participate in this process by completing a form in the surgery and handing it in at the main reception desk. This form is also available online below. Your views are always greatly appreciated and do help us to make improvements and tailor our services to your needs.
From time to time, we intend to publish some results on this website and we may refer to views expressed by patients. However, we will not publish the names of any patients or staff.
If you are happy for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.

From the 1st December 2014 it is a contractual requirement that all GP practices off the NHS Friends & Family Test (FFT).
This is confidential and continuous feedback. However, this does not replace the National GP patient Survey.
The forms are available to complete on-line or when you attend the surgery. Practices submit data to NHS England monthly and the results are to be published locally on both the practice website and NHS choices.
If you wish to complete the form below you can send the completed form via this page, fax, post.
We have had some extremely good feedback and thank all the patients that have taken the time to complete the forms.
